大连理工大学2015年各类奖学金留学生年度评审细则

一、凡享受奖学金的留学生每年都要进行一次评审，审查是否具有继续享受奖学金的资格。

二、年度评审内容（本学年）

1、学习成绩：本学年的各课考试、考查成绩；完成规定研究工作的情况。

2、学习态度和考勤：是否按时去上课、按时到研究室工作，是否按时完成阶段研究工作。

3、行为表现。

4、一年来有哪些收获、不足。

三、评审程序：

1、留学生请下载、打印并填写《年度评审表》（表1），并于 5 月18 日之前将评审表提交至留学生办公室。其中，本科及硕士生须同时提交本学年内的成绩单。

2、导师请下载、打印并填写完成《年度评审表》（表2），并于 5 月18日之前将评审表提交至留学生办公室。

四、评审为“不合格”者，中止其享受奖学金资格一年；若累计两年未通过年度评审的或无正当

理由不参加年度评审的，将被取消享受奖学金资格，不得再恢复。

五、提交地点：

1、本科生：博留公寓2号楼129室（张老师/电话：84707726）

2、研究生：博留公寓2号楼124室（付老师/电话：84770162）

The Annual Review of Scholarship Status, 2015

1.    In accordance with the Measures for the Annual Review of CSC Scholarship Status, scholarship students must participate in the annual review of scholarship status.

2.    Contents for the Annual Review of Scholarship Status (In this academic year)

① The Academic Record for each subject in the academic year; Performance in the research study;

② Learning Attitude & Attendance;

③ Behavior & Performance;

④ Summary including what you have learned or acquired, performance in the academic year.

3. Procedure for the Annual Review

   ① Scholarship students please download and complete the FORM FOR ANNUAL REVIEW OF SCHOLARSHIP (FORM 1). And please submit FORM 1 to International Students Office before 18 MAY 2015. The undergraduate and master students need submit the transcript of this academic year as well.

   ② Scholarship student’s supervisor please download and complete the FORM FOR ANNUAL REVIEW OF SCHOLARSHIP (FORM 2). And please submit FORM 2 to International Students Office before 18 MAY 2015.

4. For those who fail in the annual review, their scholarships will be suspended for the following academic year; for those who fail in the annual review for two years or fail to participate in the annual review without reasons, the scholarships will be canceled forever.

5. Where to submit? :

For undergraduate students:

Room 129 in the No.2 International Students Dormitory (Ms. ZHANG/Tel: 84707726)

For graduate students:

Room 124 in the No.2 International Students Dormitory (Mr. FU/Tel: 84770162)
