Applicants under the age of 18, please upload the following to the application system:
1.Please download and fill in the attached form. The guardian must be an adult residing in Dalian.
2.Scanned full information pages of parents’ passports (scan both fully opened pages).
3.Birth certificate. If the document is not in Chinese, a notarized Chinese translation is additionally required.
